The information you provide will help us to choose the most suitable doctor, nurse or health professional to help you. 

Please assist our Care Navigators by answering their questions fully. All our Care Navigators are fully trained and governed by the strictest of patient confidentiality requirements.

Sickness Certificates (Fit Notes)

You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home Visits

Please be aware that our GPs will always call to assess the home visit request first.

If you need a home visit, you can help us by calling reception.

House visits are only available for patients that are confined to home.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.

Change or Cancel an Appointment

Please give us as much notice as possible so we can offer your appointment to someone else.

Out of Hours

Enhanced Access

We have evening appointments on Mondays for patients who find it difficult to come in during the day.

  • Monday: 6:30pm to 8pm

Patients will be required to go to straight to waiting area B as reception will be closed after 6:30pm.

Ask at reception if you require further information.

Life Threatening

Call 999 or go to A&E now if:

 
  • you or someone you know needs immediate help
  • you have seriously harmed yourself - for example, by taking a drug overdose

A mental health emergency should be taken as seriously as a medical emergency.

Urgent But Not Life Threatening
